As you enter the central crypt, the familiar rasping voice comes from a huge shambling Folna.

Place a Shambling Brute, but do not add the usual Writhing limb that the unwounded card tells you to add.
Instead add a Sapping Spirit

If the monsters are defeated, instead of the usual reward, gain any single level appropriate item as reward, but only including legendary items if this is your first time completing this adventure.
